APPOINTMENT BOOKING AND CANCELLATION POLICY
Luxmedica requires a deposit to secure appointments with our specialists. We have two types of deposits:
• Standard deposit: £50
• Deposit for telephone consultation: £120
Please provide the full name of the PATIENT who will attend the appointment.
Cancellations or rescheduling must be made at least 48 hours before the scheduled appointment. If the appointment is not cancelled within the required time or the patient does not attend, the deposit will not be refunded.

- Photo ID (passport or driving licence)
- Any previous medical records, test results, or referral letters
- A list of current medications
- For D4 driver medical: your driving licence
